Work-Related Asthma Provider Education Program
The Provider Education Program (PEP) is leading the Work-Related Asthma (WRA) Provider Education Program intended to provide primary care healthcare professionals with up-to-date guideline-based continuing medical education on Work-Related Asthma. Education may be through workshops and problem-based self-learning modules.

Dr. Christopher Licskai, FRCPC is a Professor, Department of Medicine, at the University of Western Ontario.He is currently on active staff as a Pulmonary Medicine Consultant at London Health Sciences Centre and St. Joseph’s Healthcare in London, Ontario.He became a Fellow of the Royal College of Physicians and Surgeons of Canada in June of 1993.Dr. Licskai completed his post-graduate training in Internal Medicine and Respiratory Medicine in Toronto.His major research interest is airways disease and clinical practice guideline implementation. His major clinical interest is in asthma.Dr. Licskai is currently the Co-Chair of the Provider Education Program of the Ontario Thoracic Society.
Dr. Sharon Dell is a Pediatric Respirologist and Senior Associate Scientist at Sick Kids Hospital, Associate Professor in the Department of Pediatrics and Associate Director of the Clinical Epidemiology & Health Care Research graduate degree program at the University of Toronto where she is active in teaching and supervising graduate students. Dr. Dell’s research program focuses on childhood airway disease epidemiology and outcomes. She is also an active member of the Ontario Lung Association (OLA), Ontario Thoracic Society (OTS) and Canadian Thoracic Society (CTS), and volunteers on many committees including the OLA’s Provider Education Program, the OTS Executive and the Canadian Thoracic Society Asthma Guidelines committees.
Dr. Itamar Tamari is currently on staff at St. Joseph’s Health Centre and has been working at Stonegate Community Health Centre since 1998. Dr. Tamari completed his doctorate in Medicine at the University of Toronto in 1992 and his training in Family Medicine in 1994. Over the last eight years Dr. Tamari has chaired the Advisory for the Primary Care Asthma Program (previously the Primary Care Asthma Pilot Project) which is one of the Asthma Plan of Action (APA) projects. He sits on the Ontario Lung Association’s Provider Education Program Steering Committee and the Asthma Advisory Committee. He is also a member of the Medical and Scientific Committee of the Asthma Society of Canada and a founding member of the Primary Care Respiratory Alliance of Canada.
Dr. Ryan Smith, BASc, MSc, MB BCh BAO, RCPC, is a practicing consultant pediatrician in Orillia, ON. He obtained a degree in Applied Sciences in Chemical Engineering in 2005 and a Master’s of Science in Pharmaceutical Sciences prior to starting medical training in Cork, Ireland in 2007. He recently completed his residency at the Hospital for Sick Children in Toronto and is now Associated Staff Pediatrician at Orillia Soldiers Memorial Hospital. Dr. Smith has a strong interest in Pediatric Asthma. He has been first author on two published studies in the area and has been a requested reviewer for international journals on pediatric asthma and asthma and endocrinology. He also serves as associate editor for Paediatrics and Child Health, the journal of the Canadian Paediatric Society. More recently, he has joined as a consultant for two provincial committees on Asthma, supported by The Lung Association-Ontario and he will be taking over the position of Medical Director for the Regional Pediatric Asthma Clinic at Orillia Soldiers’ Memorial Hospital.
Dr. Tony D’Urzo is the Director of The Primary Care Lung Clinic in Toronto and an Associate Professor, Faculty of Medicine, University of Toronto, Dept. of Family and Community Medicine. I have published a number of articles in the area of Asthma and COPD management. I am a member of the Canadian Asthma Guidelines Committee (Adult and Pediatric), The Canadian COPD Alliance Steering Committee and the Primary Care Asthma Project, Spirometry Interpretation Committee: Ministry of Health and Long Term Care and The Ontario Lung Association.
Ms. Jennifer Olajos-Clow is a Nurse Practitioner and Certified Asthma Educator at Kingston General Hospital. Jennifer has been involved with the Lung Association both as a volunteer and staff over the past 5 years. She has been involved in the revision of the Primary Care Asthma Care Map and is also working with the Asthma Research Unit at KGH. She is the former provincial coordinator of the Ontario Lung Association’s Emergency Department Asthma Care Program and continues to be involved with the program as a consultant for both the adult and pediatric ED Asthma Care Pathways. She has collaborated with the Registered Nurses’ Association of Ontario in the development and publication of both the Adult and Pediatric Asthma Best Practice Guidelines for Nurses and is on faculty with the Queen’s University School of Nursing at the lecturer level. She is also a member of the Canadian Thoracic Society’s Canadian Respiratory Guidelines’ Dissemination and Implementation Committee.
Samir Gupta is a clinician-investigator, an adjunct scientist at the Li Ka Shing Knowledge Institute of St. Michael’s Hospital, and an assistant professor in the Department of Medicine at the University of Toronto. He completed a fellowship in Knowledge Translation after his Master’s degree in clinical epidemiology. His main research interests are in rare lung diseases and in knowledge translation in primary care across the spectrum of respiratory illness, with the use of electronic tools in particular, and in the area of asthma in particular. He serves as a Vice-Chair of the Canadian Respiratory Guidelines Committee.
Mr. Lawrence Jackson, is a pharmacist currently working at the Veterans Centre of Sunnybrook Health Sciences Centre. His areas of clinical specialty include: respiratory medicine, general internal, and geriatrics. He is a lecturer and seminar leader in the undergraduate therapeutics course at the Leslie Dan Faculty of Pharmacy for the topics of asthma and COPD. Mr. Jackson has also been a member of the ORSC for many years, serving on the editorial board of its newsletter, Update, and is a past co-editor. He has also served as Chair of the Ontario Respiratory Care Society (ORCS). He is a member of the Asthma/COPD Provider Education Program Committee and the Asthma Action Committee of the OLA.
Dr. Dhenuka Radhakrishnan is a pediatric respirologist at the Children’s Hospital of Eastern Ontario and Assistant Professor of Pediatrics at the University of Ottawa. She is an executive member of the Ontario Thoracic Society and has participated in the development of provincial and national guidelines for the diagnosis and management of childhood asthma. Her research interests include the use of health administrative data to investigate predictors and outcomes of severe asthma in childhood.
Ann Bartlett is the Clinical Nurse Specialist at the Firestone Institute for Respiratory Health / McMaster University St. Joseph’s Healthcare Hamilton (SJHH) Ontario. She received her Master of Science (MSc) degree in Respiratory Care at Oxford University and is one of the researchers profiled in the 2011 issue of the ORCS’s Research Review publication. In 2011, she received the A. Les McDonald Award for Innovation and Excellence in Respiratory Care and Education by the Canadian Network for Respiratory Care (CNRC). She is a part-time Assistant Clinical Professor in the School of Nursing, Faculty of Health Sciences at McMaster. Her areas of clinical practice are in allergy, asthma, COPD, and patient education. She is also the Nursing Coordinator for SJHH Respiratory Rehabilitation Programs.
Ann has co-authored with the Registered Nurses’ Association of Ontario in the development and revisions of the Adult Asthma Care Guidelines for Nurses: Promoting Control of Asthma and Nursing Care of Dyspnea: The 6th Vital Sign in Individuals with Chronic Obstructive Pulmonary Disease (COPD) Best Practice Guidelines. She has been actively involved in presenting at many local and national conferences and writing articles for various respiratory publications.
For the past 27 years, Ann has volunteered for the ORCS South Central Ontario Region and is a founding member of the Respiratory Health Educators Interest Group Executive Team. She has served on many CNRC committees and currently serves as a member of its Board of Directors. As a past facilitator and tutor in The Michener Institute’s Asthma Educator Program, Ann was part of the team that developed the Michener COPD Educator Program. She now facilitates and tutors RespTrec’s Asthma and COPD courses.
Dr Anthony J Levinson is a full-time Associate Professor, member of the Program for Educational Research and Development (PERD) and the Director, Division of e-Learning Innovation for the Michael G. DeGroote School of Medicine at McMaster University. One of the youngest endowed research chairs in the country, his work as the John R Evans Chair in Health Sciences Educational Research focuses on the integration and testing of new learning technologies to improve knowledge translation, health education and training for health care professionals and the public. He holds several grants and awards, and has co-authored one of the seminal systematic reviews of web-based learning published in JAMA, in addition to other publications on medical education. He was the Chair of the e-curriculum working group of the Council of Ontario Faculties of Medicine (e-COFM), and continues to Chair the provincial e-Learning Interest Group, and is a member of the Association of Faculties of Medicine of Canada’s Informatics Resource Group and Health Education Commons Steering Committee. Together with Dr Sarah Garside and his research and development team, he also directs the School of Medicine’s medportal, the online Continuing Health Education portal machealth.ca, as well as the patient education website machealthpublic.ca.
Dr. Diane Lougheed, is a Professor in the Departments of Medicine (Division of Respirology), Physiology and Community Health and Epidemiology at Queen’s University. She is the Director of the Kingston General Hospital Asthma Program and Medical Advisor of the Asthma Education Centre. Dr. Lougheed is an Adjunct Scientist at ICES-Queen’s. She obtained her medical degree from McMaster University, and completed her Internal Medicine residency at McMaster University and Respirology fellowship at Queen’s University. She also has a Master’s Degree in Epidemiology. Her research interests include: asthma symptom perception and outcome research.
Jennifer Mackinnon is a Respiratory Therapist and Certified Respiratory Educator with a background that includes twenty years working in respiratory home care. During this time she had the opportunity to lead healthcare teams and implement many respiratory health programs and services. In 2016, she joined The Lung Association’s Provider Education Program team as Curriculum Developer and is currently the Program Manager. She has a strong interest in promoting respiratory health and her interests include education, motivational interviewing, and quality improvement.
Lana Biro is a Registered Respiratory Therapist and a Certified Respiratory Educator at The Lung Association – Ontario. She has a strong interest in promoting respiratory health and has been with The Lung Association for over ten years, answering calls on the provincial Lung Health Information Line, providing counselling and guidance to patients, caregivers and health-care providers on a variety of lung health issues, such as asthma, COPD, smoking cessation and air quality.
Lana is a trainer for RESPTREC®, facilitating workshops for health-care professionals on asthma, COPD, Spirometry and Education for Chronic Disease. She is also a CAMH/TEACH-trained facilitator for Motivational Interviewing workshops.
Lana has been involved with various lung health advocacy initiatives, including work with the Lung Health Ambassadors, Breathworks®/COPD newsletters and Lung Health/COPD support groups in Ontario. Lana enjoys organizing in-services and educational presentation for health care providers, as well as participating in various educational opportunities.
Dr. Joe Reisman completed his medical degree degree at the University of Toronto in 1980 and an MBA at the University of Toronto’s Rotman School of Management in 1998. His postgraduate medical training includes a mixed internship year at Mount Sinai Hospital in Toronto from 1980 to 1981 and a year of general surgery at the University of Ottawa from 1981 to 1982. He completed a three-year residency in general pediatrics in 1985 and a three-year fellowship in pediatric respirology at the Hospital for Sick Children (SickKids) in Toronto. In 1988, he joined the staff of the Division of Respiratory Medicine SickKids. From 2000 to 2011, he was the chief of Pediatrics at the Children’s Hospital of Eastern Ontario (CHEO) and a professor and chairman of the Department of Pediatrics at the University of Ottawa. His research focus has pertained to clinical trials in pediatric asthma and cystic fibrosis.
Andrea Stevens Lavigne, Vice President, Provincial Programs
Andrea Stevens Lavigne was appointed Vice President, Provincial Programs for the Ontario Lung Association (OLA) in February 2010. In this position, she is responsible for the Association’s respiratory, tobacco-related and air quality programs, and the work of the Ontario Thoracic and Ontario Respiratory Care Societies. With more than twenty years of strategic leadership and management experience in a provincial agency, a hospital and a primary health care organization, Andrea brought to the position a passion for health promotion, knowledge exchange and high quality community services.
A graduate of the University of Toronto with an MBA, Andrea held various management roles at regional and provincial levels with the Centre for Addiction and Mental Health (CAMH) and the Addiction Research Foundation (ARF) prior to its merger with CAMH. As a Director and Acting Vice President at CAMH, she was directly responsible for the development, implementation and evaluation of provincial knowledge exchange and health promotion programs, working with diverse teams across 26 sites in Ontario. She has managed complex projects involving multiple stakeholders, aimed at increasing capacity of professionals to deal with the prevention and treatment of alcohol, tobacco and other substance misuse, as well as mental health issues.
Prior to joining OLA, Andrea was the Executive Director responsible for the establishment of the Etobicoke Medical Centre Family Health Team. In this role, she led the development of the organization’s first strategic plan, established internal policies and procedures, recruited a range of health practitioners and initiated health promotion programs.
Experienced in government relations, Andrea has served on the Toronto Drug Strategy Strategic Advisory Committee, the Ontario Ministry of Health and Long-Term Care Public Health Technical Advisory Committee and has also contributed to national level initiatives related to alcohol policy and mental health promotion.
Gloria Alfred is the Training Coordinator for the Provider Education Program (PEP) and the Ontario Respiratory Care Society at The Lung Association. Her responsibilities include coordinating conference exhibits and continuing medical education events across Ontario and assisting with the development and evaluation of these programs. Gloria obtained her certificate in E-learning at the University of Toronto and holds a degree in Sociology from the University of Waterloo.
Carole is the Director of Respiratory Health Programs, for the Ontario Lung Association. She is responsible for the delivery of five asthma programs funded by the MOHLTC as part of the Ontario Asthma Plan of Action. Her responsibilities in this position include marketing, media initiatives, The Lung Association’s BreathWorks COPD programs and participating on several respiratory advisory committees. Carole graduated from The Michener Institute in Toronto as a Respiratory Therapist in 1981 and she holds a Master of Applied Science degree. Prior to working with the Ontario Lung Association, Carole worked for 9 years as the Team Leader in a respiratory rehabilitation program. She also worked for 26 years as an acute care RRT. Carole has presented respiratory related topics at numerous educational events and seminars. Carole has published articles in respiratory journals, in newspapers and she has been interviewed on radio and television.
Sylvie Bouchard is one of the Program Administrators for the Provider Education Program (PEP) for the Ontario Thoracic Society at The Lung Association. Her responsibilities include coordinating conference exhibits and continuing medical education events across Ontario and assisting with the development and evaluation of these programs. Sylvie holds a Honours Degree in Health Studies from York University and is bilingual in French and English.
Dr. Chris Allenis a Respirologist at the Firestone Institute for Respiratory Health in Hamilton Ontario and an Associate Professor of Medicine at McMaster University. His medical training was at Oxford University and the Royal London Hospital. After further general medical training at the London Hospital, which led to the MRCP diploma, he worked in Bath England and completed training in Respiratory Medicine at McMaster. He joined the staff of the Firestone Clinic in 1984. Dr Allen’s research interests have been in respiratory disorders related to gastroesophageal reflux and he also has a long-standing interest in program evaluation and medical database development.
Dr. Susan Waserman obtained her MSc and medical degree from McGill University in 1981. She specialized in Internal Medicine, followed by subspecialty training in Allergy/Clinical Immunology, completing fellowships at the Meakins Christie Laboratories at McGill University in Montreal, and at McMaster University. She is a past President of the Canadian Society of Allergy and Clinical Immunology. In this position her activities include continuing medical education for physicians, allied health professionals and the community, as well as developing practice guidelines in areas such as anaphylaxis, and immunotherapy. Her research interests and activities include clinical trials in a variety of therapeutic areas related to allergy, including rhinitis, asthma, as well as determinants of allergic expression in asthma and food related anaphylaxis.
Dr. Monique Forse is a Respirologist. She is a graduate from McGill University Medical school and trained in Internal Medicine and Respirology at the University of Toronto. She has worked at the Markham Stouffville Hospital for 25 years. She established the Centre for Respiratory Health where an Asthma education program, COPD clinic, sleep lab and CPAP clinic are located. She helped to develop the hospital respiratory diagnostic services which include pulmonary function, methacholine, exercise asthma and cardiopulmonary exercise testing. She continues to be actively involved in teaching respiratory medicine to physicians and healthcare professionals.
Gemma is a Registered Respiratory Therapist and Certified Respiratory Educator. Along with her experience in delivering acute RT care in the hospital setting, she has worked in several areas of outpatient clinics including, Pulmonary Function Labs, Sleep Clinics, COPD and Paediatric Asthma Clinics . She is a neonatal resuscitation instructor and is now with the Ontario Lung Association, Provider Education Program, focusing on curriculum and program development. Gemma is also the provincial co-ordinator for the Emergency Department Asthma Care Pathway across Ontario. As a provider education program developer, she works on implementation and evaluation of continuing medical education programs and materials on respiratory disease across the province, with a keen interest in program evaluation and respiratory research.
Natasha Khalfan is the PEP Training Coordinator with the Lung Association. Natasha graduated from the University of Toronto; she holds an Honours BSc with a major in Health Studies as well as a Masters in Public Health specializing in Health Promotion. She is responsible for the planning and coordination of local and regional training programs and events throughout Ontario. Natasha has a background in planning, implementing and evaluating projects specifically in the areas of Chronic Disease Prevention and Management. Her experiences also include engaging with Health Service Providers in the community with respect to integrating regional primary care services.
